Au Japon, dans un futur proche, le vieillissement de la population s’accélère. Pour y remédier, le gouvernement met en place le programme « Plan 75 » qui propose aux citoyens de 75 ans et plus un accompagnement logistique et financier pour se faire euthanasier. Une femme âgée, Michi, un chargé de recrutement du plan 75, Hiromu, et une jeune aide-soignante philippine, Maria, se retrouvent confrontés au choix de la vie et de la mort.
